In addition to my informed consent, I fully agree to terms and conditions associated with this procedure as discussed with my therapist. I voluntarily agree to undertake the treatment, fully agree with Terms & Conditions associated with this treatment and agree that Beauty Salon cannot be held responsible for any allergies / physical reaction as result of the treatment, and I have in addition, signed to demonstrate that I am fully satisfied with my results during and after my treatment.

I have also been informed and have undertaken the patch test and observed a 48 hours waiting period after patch test has been performed. I agree to the terms and conditions of this disclaimer form and I am in agreement that Beauty Salon cannot be held responsible for any allergic / physical reaction as result of the treatment.
Therefore there will be no consequent fault by V & O Salon and I cannot request any monetary or complimentary service repercussions (whether monetary or complimentary service / treatment reparation, refunding or compensation) to be made to myself by V & O Beauty Salon after I have been treated and completed the treatment on this day, (Please refer to the aforementioned date).
